Documentation

To start the registration process with UAF Disability Services (DS) and to receive reasonable accommodations while attending UAF, documentation indicating that a disability limits a major life activity must be submitted to our office. 

Acceptable documentation should:

  • state the symptoms and impacts you are experiencing, the disability or the diagnosis.
  • be recent, ideally within the past three years. However, this is not necessary for documented life long disabilities. 
  • be signed by your medical provider, or submitted by a prior institution that you have attended. 

Additionally, if you wish, you can have your provider fill out and sign the Disability Verification Form. Please note that a completed Disability Verification Form is sufficient medical documentation to register with DS, and further documentation is normally not required. 

DS reserves the right to request additional documentation from the student, or to request clarifying information from the provider.
